Job Summary:

The Care Coordination Support Specialist / Patient Care will provide administrative and patient support to the Care Coordination team to help ensure efficient delivery of patient care services. This role involves assisting with appointment scheduling, patient follow-ups, documentation, and coordination of resources as directed by licensed Care Coordination staff. The position requires strong communication, organization, and interpersonal skills to effectively support patients, families, and healthcare providers.

Key Responsibilities:

  • Provide administrative assistance to patients and Care Coordination staff in supporting patient care plans
  • Assist in scheduling patient appointments and coordinating care services as directed by Care Managers
  • Help connect patients and families with appropriate community resources and healthcare services
  • Maintain accurate and detailed progress notes for patient interactions
  • Prepare reports and maintain documentation required by the Care Coordination team
  • Assign Care Managers to referred patients and assist with initial and follow-up scheduling
  • Send written communication to patients and confirm service appointments as needed
  • Assist patients with navigating insurance requirements such as prior authorizations and out-of-network waivers
  • Escalate clinical concerns or decision-making requirements to Care Managers when necessary

Qualifications:

  • Bachelor’s degree in Business, Healthcare, or related field (required)
  • At least 1+ year of experience in a community, ambulatory care, or healthcare-based setting (required)
  • Experience working with patients, families, and healthcare providers (required)
  • Experience in case management in ambulatory or transition care settings (preferred)
  • Previous experience in healthcare, BPO, or shared services environment (preferred)

Technical Skills:

  • Ability to review and synthesize patient care plans and resource requirements to provide quality service
  • Experience using Electronic Medical Records (EMR) systems is preferred
